Super Shot

It's free, it's for kids, it's for life!

The Super Shot program comes to Emanuel on the fourth Thursday of each month, 4:00-7:00 p.m.

* Super Shot provides free immunizations for kids from birth through age 18.

* Call (260) 424-7468 for further information.

* Please bring you children's shot record.

* Child must be accompanied by a parent, legal guardian or have a presigned consent form that you can get at any Super Shot Clinic.

* These clinics provide the Chickenpox vaccine. Documentation of disease history or vaccination is mandatory for kindergarten and 1st grade.
